If your child is a new patient, we request that your child’s medical records be sent to us prior to the appointment. Please complete the Release of Records to Foundation Medical Partners (pdf) form to release your child's medical records. To schedule an appointment, please call the number at left that corresponds to the specialty you need.

New Patient Registration
Before making an appointment, new patients should contact Mass General for Children at 888-644-3248 to receive a patient number. Please have the following information available:
Name
Address
Date of birth
Social security number
Insurance information
Emergency contact information
Many insurance companies require a referral before seeing a specialist. Please contact your child's primary doctor to request a specialty referral. If a referral is not on file when you check-in, we will ask you to sign a financial waiver as a guarantee of payment.
What do I need to bring with me to my appointment?
We request that you bring your current medical insurance card to each appointment, as well as any co-pay that your insurance company requires. If you are a new patient, please be sure to bring your new patient paperwork.
Who can bring my child to the office for treatment?
We strongly recommend that a parent or legal guardian accompany your child to office visits. A written consent form must accompany your child or be on file when a parent or legal guardian is unavailable.
Checking In
New patients should plan to arrive at least 10 minutes before their scheduled appointment to complete necessary paperwork.
Cancellations and Late Arrivals
It is extremely important to keep your appointment and to arrive on time. If you need to reschedule, please call 603-577-5360 at least 24 hours in advance of your appointment.

Please request prescription refills at the time of your office visit. If you need to refill a prescription between office visits please call Mass General for Children at:
