Access to Resources for Community Health (ARCH)

The ARCH program connects residents and professionals of Charlestown, Chelsea, Everett and Revere with high-quality, health-focused information services.

Adolescent Health Initiative

The MGH Revere Adolescent Health Initiative provides health services for young people ages 10 to 21 in Revere in order to promote healthy behaviors, reduce risk, and provide or refer patients to appropriate interventions.

Boston Health Care for the Homeless Program (BHCHP) at Massachusetts General Hospital

Boston Health Care for the Homeless Program assures access to the highest quality of care for homeless men, women and children in metropolitan Boston. Multidisciplinary teams at BHCHP deliver direct care services in over 78 sites, including a site located within Mass General.

Boys and Girls Club Partnership

Mass General has partnered with the Boys and Girls Clubs of Boston (BGCB) to provide nursing staff and a community health specialist to the staff and youth participants of the Boys and Girls Clubs of Boston. The staff focus on providing nursing services and health education to all of the Boys and Girls Clubs, as well as summer camps provided by BGCB.

Cancer Navigation Program

Patient navigators work with vulnerable patients needing help to receive preventive cancer screenings, make it to diagnostic specialty appointments, and cancer treatment.  Patient navigators specialize in addressing patients’ barriers to care, ranging from transportation to health literacy to fear.  The navigators provide reminders, health education and support, and may accompany the patient to an appointment if needed.

Charlestown Coalition

The Charlestown Coalition (formerly the Charlestown Substance Abuse Coalition) was formed in 2004 by residents and community partners in response to the devastating opioid epidemic and overdose related deaths among young people in the neighborhood. Their work has expanded to go beyond substance use and focus additionally on stable housing, healthy food, racial equity, and access to mental health services.

Charlestown Family Support Circle

The Family Support Circle is a program of the Charlestown Coalition. The Family Support Circle assists youth and families by connecting them to resources and supports in and around the community. They provide short term counseling, case management, and care coordination services to residents.

Complex Patient Population Community Health Worker Program

Community Health Workers (CHWs) work with patients who have multiple health issues and experience barriers to care.  Most CPP patients are immigrants or refugees who have limited English proficiency, little social support and/or are not familiar with the US medical system. CPP CHWs meet patients where they are at in their care, help create and accomplish goals, and ultimately increase their connection to primary care. 

Connect to Wellness

Connect to Wellness offers on-site health and social services to residents living in three apartment buildings surrounding the hospital campus in Boston’s West End and Beacon Hill. The objective of this community collaborative is to assist all residents in maintaining independence as they age in place by identifying social and health related needs and providing intervention.

EASTIE Coalition

The EASTIE Coalition works to strengthen protective factors and decrease risk factors to prevent substance use and abuse for youth, adults and families through education, prevention, and intervention strategies.

Family Planning Program

The Family Planning Program provides confidential reproductive health services to adolescents, young women and men.  It ensures delivery of clinical family planning services at Mass General Revere Pediatrics, Mass General Revere School-Based Health Center, Mass General Revere Adolescent Health Center, Mass General Chelsea Pediatrics, and Mass General Chelsea School-Based Health Center.

Food for Families

Food for Families identifies pediatric patients and their families in Chelsea who have experienced hunger and food insecurity and connects them to food resources.

Hepatitis C Program

The program works to improve clinical care and increase the screening and treatment of Hepatitis C Virus (HCV) through treatment and patient education, and community outreach activities.

Helping Abuse & Violence End Now (HAVEN)

For Mass General patients, employees, and community member who have experience intimate partners abuse, HAVEN offers direct services to help improve survivors’ health and safety, as well as their housing, employment, and financial circumstances.

Healthy Chelsea

Healthy Chelsea is a community coalition that is focused on improving the overall health of Chelsea residents by working to increase healthy eating/active living and to decrease the negative effects of substance misuse and toxic stress.

Healthy Families America Home Visiting Program

Healthy Families America program provides home visiting services to vulnerable families to support healthy child and family development from pregnancy through 3 years of life.

Healthy Steps Family Home Visiting Program

Program provides timely well child visits/immunizations and increased parental knowledge of child development, healthy eating habits and obesity prevention. In addition, the program seeks to improve access to care for all patients and their families.  Child development specialists in the Healthy Steps program conduct joint office visits with the pediatricians during well child checks for children between the ages of birth and three years. Healthy Steps is offered to all first-time parents bringing their newborns to Mass General Revere for pediatric care. 

Living Tobacco-Free Program

The Living Tobacco-Free Program works with Mass General community healthcare centers and community organizations to help people stop using tobacco products and to prevent youth and others from initiating tobacco use.

MGH Youth Programs

MGH Youth Programs aim to educate young people in Boston from elementary through high school about health and science subjects and expose them to related careers in order to equip them with the knowledge and skills to excel in life and in their careers.

Medical Interpreters and Community Health Workers

The Medical Interpreters/Community Health Workers (MI/CHW) program aims to reduce barriers to health care for MGH Chelsea patients with limited English proficiency by providing high-quality linguistic and health system navigation services.

Pediatric Asthma Program

An asthma worker provides health education through in office visits, home visits and group teaching sessions. The patients’ understanding of chronic asthma and how to control it with daily medication is reinforced through follow-up and assistance managing the triggers in the home. 

Police Action Counseling Team (PACT)

PACT provides crisis intervention to children who witness or have been victims of violence to reduce trauma and to ultimately interrupt the cycle of family violence.

Prenatal Outreach Program

The Prenatal Outreach Program provides support, health education and referrals to the diverse Chelsea patient population during pregnancy and up to the first six weeks postpartum in order to ensure a healthy pregnancy and engagement in primary care for both mother and child. The Prenatal Outreach Program also encourages and engages fathers in prenatal care.

Revere CARES

The goal of the Revere CARES coalition is to improve the well-being of Revere’s residents by preventing substance abuse and promoting healthy eating, active living, and healthy relationships.

Revere Youth Zone

The Mass General Revere Youth Zone (YZ), located at 300 Broadway, is a no-cost afterschool program and at-risk-youth, 9-17 years of age.

School-Based Health Center Program (SBHC)

Staffed with Pediatric Nurse Practitioners and other clinicians, SBHCs provide primary care and services in mental health, nutrition, health education, substance abuse prevention, violence prevention, and assistance with health insurance enrollment to all students at Chelsea High School and Revere High School.  

Smart Choices Program

The Smart Choices Program offers annual grants to help Charlestown organizations meet the health and human service needs of the community.

Stay in Shape

Stay in Shape is a free after-school program designed to promote awareness and inspire healthy living to prevent childhood obesity among adolescent students in the communities served by Mass General HealthCare Centers.

Suboxone Program

The Suboxone Program provides nursing case management and support for patients with substance use disorders, specifically opioid addiction. This program provides an innovative approach to substance use disorder treatment within the primary care practice.

Violence Intervention Advocacy Program (VIAP)

A VIAP advocate helps guide victims of community violence through recovery from physical and emotional trauma, empowering them to make positive changes in their lives.
