Connect to Wellness offers on-site health and social services to residents living in three apartment buildings surrounding the hospital campus in Boston’s West End and Beacon Hill. The objective of this community collaborative is to assist all residents in maintaining independence as they age in place by identifying social and health-related needs and providing intervention.

Our Services
Wellness Nurse Services
- Provides an initial resident assessment to determine unmet care needs and triages referrals to team members
- Coaches residents in the management of chronic conditions
- Assesses blood pressure, weight and provides basic first aid
- Assists residents with communicating to their health care team
Community Health Worker Services
- Works directly with residents to identify barriers to healthcare and provides resources, information and education
- Teaches and facilitates educational classes on various health topics
- Identifies and shares helpful resources for building management, staff and residents
Community Outreach Coordinator Services
- Coordinates health and wellness programs based on the needs and interests of building residents
- Researches new and existing resources and programs in the community and within Mass General to bring to the buildings
