About the Programs

We started with the PATH Program, focusing on structured education and personalized coaching to help young transplant patients transition smoothly from pediatric to adult care. Over time, we recognized the immense value of peer support and launched Peer Connections, offering in-person and virtual events that foster community and resilience. During the COVID pandemic, we introduced interactive Zoom events, mailing out craft kits and hosting Q&A sessions, which evolved into GOAT, a series where patients gain health and life skills with opportunities to connect with peers and ask questions to providers.

Each program uniquely supports young transplant recipients, yet they work together to ensure patients have the comprehensive support they need at every step of their journey.

Going On After Transplant (GOAT)
