Making Referrals to the Multiple Sclerosis Clinic
We appreciate you entrusting us with caring for your patient with known or suspected demyelinating disease. We are accepting new patients for longitudinal care and one time second opinions. New patient referrals for evaluation of known or suspected diagnoses of multiple sclerosis, neuromyelitis optica spectrum disorder, MOG antibody disease, optic neuritis, or transverse myelitis can be directed to our office by phone (617-726-7565) or by fax (617-724-0895).
We request that all pertinent clinic notes, MRI reports, and laboratory testing be faxed to our office at at 617-724-0895.
To ensure that all prior imaging is available for review at the initial appointment we request that new patients either bring a copy of their prior MRIs on CD to their appointment or have these mailed to our office in advance of their appointment to be uploaded to their medical record.
