We specialize in primary care of older adults with the goal of emphasizing preventive care and health maintenance. Our physicians work in teams with nurse practitioners, social workers, case managers as well as geriatric psychiatrists, geriatric cardiology and geriatric urology. We are particularly interested in helping manage the challenges facing patients with multiple chronic conditions, functional limitations and cognitive impairment and their families.
Inpatient Services
If you require a hospital admission to Mass General, we provide close collaboration with the admitting attending physician. There are circumstances, like an emergency, when the ambulance will take you to the closest hospital for evaluation and stabilization. If appropriate and possible we would be happy to assist in your transfer to Mass General.
Mass General is a teaching hospital, and we are pleased to work in collaboration with physicians in training (doctors who are pursuing additional training in Geriatric Medicine or Internal Medicine) who may see you in addition to our staff.
Upon discharge from the hospital we request that you make a follow -up appointment with your physician or our nurse practitioners. Please call us at 617-726-4600.
Geriatric Psychiatry
Our practice offers the expertise of a geriatric psychiatrist who works with our primary care physicians to diagnose and treat conditions such as depression, memory problems, dementia, anxiety, stress, coping with chronic disease and terminal illness. Services include psychopharmacology, psychotherapy and consultation to the team.Social Services
Geriatric social workers are integral members of the team who work with patients, family members and caregivers. Services include individual counseling for patients, caregiver support and care coordination. Areas of discussion may include home health care, nursing home and assisted living options, caregiver stress and coping with the emotional challenges of aging.
