Referring Physicians
Referring a patient to the Pediatric Neuromuscular Clinic is simple.
You may either contact clinic coordinator Laurie Bliss directly at 617-840-0528 or fill out a Referring Physician form and fax it to Laurie at 617-724-7860. Please ask the parent or guardian to call the Registration Office at 1-866-211-6588 to ensure registration information is correct and up to date prior to the appointment.

Please have parents/guardians contact their insurance companies for any preauthorizations and/or referrals.
International patient families may contact the International Patient Center at 617-726-2787, which will assist them in registering their child and help with financial concerns.
